 Investor Identification Form

This form is used to gather the necessary information to enable BlackRock to electronically verify the identity of an investor.


 Foreign Corporate Entity Form

This form is used to gather the necessary information to enable BlackRock to verify the identity of a foreign corporate investor.


 Who can certify AML documents

This form provides a comprehensive outline on the list of persons that can certify AML documents for the purposes of the Anti-Money Laundering and Counter-Terrorism Financing Act 2006 (Cth).


 Application for additional units in existing Fund(s)

Use this form to apply for additional units in a Fund or Funds in which your client is already invested. Make sure the form is signed by the appropriate parties.


 Direct Debit Authorisation Form

If you wish to deposit your investment by Direct Debit please complete and sign this form and attach to the application form. A Direct Debit Authorisation form is required to be completed for each bank account to be directly debited.

Note: If you hold units in one Fund and wish to invest in another Fund for the first time, you will need to complete an application form attached to the relevant Product Disclosure Statement (PDS).


 Appointment of nominated representative

Appointment of nominated representative form - applies to BlackRock's Investment Funds range of products. It allows your clients to nominate a person to act as their authorised representative/ agent and operate aspects of their account with BlackRock. This includes authority to transact on their account and make changes to the account.

Please ensure that your clients read the 'how to complete' section of the form and in particular the section headed "Appointment of nominated representative" before they complete the form.

  Client change of address

Use this form to advise us of any address changes. Please ensure that your client signs it – we are unable to process this request in the absence of a client signature or a duly executed Power of Attorney.


 Confirmation of distribution instructions

Use this form to alter the method by which income distributions are paid. Please ensure that your client signs it – we are unable to process this request in the absence of a client signature or a duly executed Power of Attorney.


Regular contribution plan

This form is used to have monthly or fortnightly contributions to your fund deducted from your bank account OR to change any details of an already existing Regular Contribution Plan.

 Redemption request

Use this form to redeem some or all units on your client's behalf. Please ensure that your client signs it – we are unable to process this request in the absence of a client signature or a duly executed Power of Attorney.


 Standard transfer form

Transferring units from one investor to another requires completion of a standard transfer form. Download and complete the Standard Transfer Form (STF), as required in the particular circumstances of your client. Please ensure that both the seller and buyers signatures are completed in the appropriate sections on the form.


 Switching form

Please ensure that you follow these instructions when making a full or partial switch from one investment portfolio to another.


 Tax File Number

Investors are not obliged to quote their Tax File Number. However, if they are an Australian Resident for tax purposes and do not quote their Tax File Number, we are obliged to deduct tax at the highest marginal rate, plus the Medicare Levy, from income distributions.
