Use this form to apply for additional units in a Fund or Funds in which your client is already invested. Make sure the form is signed by the appropriate parties.
Direct Debit Authorisation Form
If you wish to deposit your investment by Direct Debit please complete and sign this form and attach to the application form. A Direct Debit Authorisation form is required to be completed for each bank account to be directly debited.
Note: If you hold units in one Fund and wish to invest in another Fund for the first time, you will need to complete an application form attached to the relevant Product Disclosure Statement (PDS).
Appointment of nominated representative
Appointment of nominated representative form - applies to BlackRock's Investment Funds range of products. It allows your clients to nominate a person to act as their authorised representative/ agent and operate aspects of their account with BlackRock. This includes authority to transact on their account and make changes to the account.
Please ensure that your clients read the 'how to complete' section of the form and in particular the section headed "Appointment of nominated representative" before they complete the form.
Use this form to advise us of any address changes. Please ensure that your client signs it – we are unable to process this request in the absence of a client signature or a duly executed Power of Attorney.
